An Advance Care Plan is a document that tells your doctor how you want to be treated if you are unable to make the healthcare decisions for yourself.
You can use an Advance Care Plan to:
  • tell your doctor whether or not you want life-prolonging medical treatment
  • add special instructions, such as burial arrangements
  • donate your organs
  • name a person to make healthcare decisions for you if you are unable to make the decisions yourself
