The Mountain States Health Alliance Wellness Program offers the opportunity to assist you to become healthier through awareness, education and resources. We are offering this program which consists of 3 steps:
Please do the following in SEQUENCE....
STEP 1 - Online Registration
Complete the Health Risk Assessment (HRA) KNOW YOUR NUMBER questionnaire

Once HRA is completed, a lab confirmation will be sent to the
email address you designate within minutes from support@biosignia.com
STEP 2 - Lab Draw
Take confirmation email FROM BIOSIGNIA and name badge to
any MSHA Facility or Synergy Lab (click here for locations) for lab draw
No appointment necessary
Remember to FAST for 12 hours
(Nothing per mouth except water and medications)
**You must have labs drawn at least 4 days prior to your scheduled appointment
STEP 3 - Screening and Consultation (allow 30 minutes)
The Screening - Blood Pressure, height, weight, and waist measurement
The Consultation - one-on-one review of results with a Health Educator
