Welcome to MSHA Vendor Certification

What is Vendor Certification?

Vendor certification is an initiative at Mountain States Health Alliance that will require all vendors to become “certified” to do business with Mountain States.
 
How do I begin this process?

The first step in becoming a certified vendor with MSHA is to complete the provided vendor questionnaire. It is mandatory that all questions on this questionnaire be answered. There are two sections on the questionnaire.
 

Section 1:Corporate” - If there is more than one representative for a specific vendor, this section will only need to be completed once.

Section 2:Personal” – Each individual representative will need to complete this section.
