What is Vendor Certification?
Vendor certification is an initiative at Mountain States Health Alliance that will require all vendors to become “certified” to do business with Mountain States. |

How do I begin this process?
The first step in becoming a certified vendor with MSHA is to complete the provided vendor questionnaire. It is mandatory that all questions on this questionnaire be answered. There are two sections on the questionnaire. |

Section 1: “Corporate” - If there is more than one representative for a specific vendor, this section will only need to be completed once.
Section 2: “Personal” – Each individual representative will need to complete this section.
