Is Home Health for Me?

Home Health involves providing care to a person in their own home. They may not need hospital care or nursing home care, but require some day-to-day health and personal needs.

Home Health may be appropriate for you or a member of your family if:

  • You are having trouble caring for yourself or a family member is having difficulty due to added responsibility, health or social problems
  • You are able to provide adequate care for yourself or loved one but feel angry, frustrated, or exhausted at the end of the day
  • You are homebound, living alone, and feel depressed, fearful, or lonely
  • You still require additional care following your discharge from a hospital or nursing home
  • You are on medications that need to be monitored, have a long-term illness, need skilled nursing care, or short-term observation due to a recent illness
  • If you have a history of recurrent heart problems or diabetes mellitus
  • You are participating in rehabilitation services to regain or improve your mobility or enable you to utilize independent living skills

Obtaining Home Health Care

The physician, in most cases, makes the decision regarding the feasibility of home health care services. When hospitalized, the hospital's nurse coordinator, social worker, or discharge planner can assist in arranging for home health care services following discharge. All services are provided in accordance with the physician's plan of care.

Health WatchTM

Personal Response Systems called Health Watch are utilized by our Home Health team at Norton Community Hospital. Accidents and medical emergencies can happen at any time to anyone. The idea behind this concept is to devise a personal response system that allows you to feel confident that help will be available in the event of a sudden emergency. Health Watch allows you to be comfortable in your own home giving you confidence that when you need help someone is always there 24 hours a day. The Health Watch Operator knows you and your vital statistics and will call your family, neighbors, doctor or emergency services if you are in trouble.
