to the Inpatient Rehabilitation Unit at Norton Community Hospital (NCH), where treatment is a patient-centered team approach involving patients, healthcare professionals and families. The inpatient rehabilitation team at NCH helps patients reach their goals of returning to home, work or school as soon as possible. Your referral is the first step in this process, affording you the best opportunity for a positive outcome and a satisfying patient experience.
At our rehab unit, knowledge and information are freely shared among patients, care partners, physicians and other caregivers. The rehab team meets regularly to coordinate care and to update goals based on patient progress. One of our Patient-Centered Guiding Principles is to customize your care to reflect your needs, values and choices. Thus, an effort is made to keep the same Team Members working with you and your family throughout your stay.
Our rehab care is customized to fit patient needs and to promote recovery from conditions such as stroke, traumatic brain injury, amputations, hip fractures and other disabling conditions. We maintain a coordinated team effort to achieve a positive outcome. All Team Members are considered caregivers. Your Team Members include a medical physician, physical and occupational therapists, rehabilitation nurses, speech-language pathologists, psychologists, case managers and dietitians. The Inpatient Rehabilitation Unit is located within the hospital setting and rehabiltiation patients have access to medical physicians and services 24 hours per day, seven days per week.
Your care will be provided in a healing environment of comfort, peace and support.
