stroke.  The goals of our program include minimizing impairments and complications, decreasing limitations to activity, maximizing quality of life, decreasing environmental barriers, and preventing recurrent stroke.  Each of the patients admitted to our program receive education, intense therapy services, and assistance with their transition to the next level of care.  As a CARF certified stroke program, our team is committed to serving the patients and families that have been impacted by a stroke.  Each of the patients admitted to our program receive education, intense therapy services, and assistance with their transition to the next level of care.  As a CARF certified stroke program, our team is committed to serving the patients and families that have been impacted by a stroke. 

Scope of Services for Stroke Specialty Program

  • The Stroke Specialty Program defines interventions in areas such as preventi8on of stroke, treatment of conditions associated with stroke, reducing risk factors for recurrent stroke, and functional independence. 
  • The Stroke Specialty Program addresses prevention of secondary complications.
  • In response to the preferences of the person served, the stroke specialty program provides information on various treatment approaches to maximize outcomes.
  • The Stroke Specialty Program offers specific education for the persons served and families/support systems that is coordinated and reinforced throughout the rehab process. 
  • The program facilitates collaboration with the person served and family/support system in decision making throughout the rehab process.
  • To meet the needs of the persons served, the stroke specialty program demonstrates knowledge and appropriate utilization of assistive technology. 
  • To facilitate transition to the community an individualized plan is established for each person served. 
  • Prior to the day of discharge/transition, the stroke specialty program coordinates post discharge care and necessary follow up appointments.
  • The Stroke Specialty Program acts as a resource for providers throughout the phases of stroke care from emergency through community-based services. 
  • The Stroke Specialty Program demonstrates its advocacy role within the community for persons who have sustained a stroke.
