Francis Marion Manor Health & Rehabilitation is a 109-bed licensed nursing care facility, an entity of Smyth County Community Hospital, which is a member of Mountain States Health Alliance. Francis Marion Manor Health & Rehabilitation provides both skilled-level and nursing facility-level health care services. A full scope of services is provided,
including 24-hour nursing services, a registered dietitian, physical and occupational
therapists, a speech language pathologist, certified activity staff,
a social worker, a beautician and medical director.
Skilled Care Services
Residents who qualify for Skilled Nursing Services are screened prior to admission.
Services related to this type of care may include:
- Physical, occupational and/or speech therapy following orthopedic surgery, stroke or accident
- Daily administration of intravenous medication
- Daily cleaning and dressing of certain wounds
- Diabetes-related care
- Strict or complicated medication regimens
Individuals admitted to the skilled care nursing unit require specialized treatment five to seven days per week and need to show continual improvement. The goal of skilled care is to assist in improving functions to prepare for a smooth transition home. As their care continues, meetings with family and patient determine the next steps in care.
Nursing Facility Care Services
Generally, admission to this unit is approved after a physician gives admission orders and an eligibility screening is performed. Both the functional capacity and care needs of the individual must be considered. Typically, individuals for whom nursing facility care is appropriate include those who need help with the following aspects of daily living:
- Using toilet facilities
- Bowel/bladder assistance
Our rehabilitation services are designed to help patients increase their independence. An interdisciplinary team approach combines the following areas:

On the day of admission to Francis Marion Manor Health & Rehabilitation, required forms must be completed and medical information provided. If the resident is unable to do so, a responsible family member or official power of attorney must be available. Bring all insurance information and a complete list of home medications on the day of admission. Prior to admission, a meeting with a social worker/case manager of the discharging facility and/or the admissions assistant of Francis Marion Manor must occur. What to Bring
Space in residents' rooms is limited, but personal
items will make a room feel more like home and can contribute to the comfort of someone entering a nursing facility. Items should include sleepwear, bathrobe, casual clothing for daily wear, comfortable non-skid shoes, and if necessary, eyeglasses, hearing aids and dentures. Do not bring medications. For the facility to administer medications, an order must be obtained from the admitting physician. Medications then will be ordered through an approved pharmacy.

From Interstate 81, take exit 47. Southbound: Turn left onto Highway 11 at bottom of exit ramp. Northbound: Turn left at bottom of exit ramp and proceed to stop sign. Turn left onto Highway 11. Follow Highway 11 to second traffic light. Merge right onto Highway 16 North/Park Boulevard. Drive approximately 0.75 miles through one traffic light. Turn right onto Radio Hill Road. Proceed and turn right onto Francis Marion Lane. Turn left into Francis Marion Manor parking lot.
