Medical: Benefit-eligible Team Members may choose from plans administered by CrestPoint Health. Preventive care and prescription drug coverage are included in these medical plans.Employee Assistance Program
Dental: Benefit-eligible Team Members have several options to choose from administered by Delta Dental Plan of Tennessee.
Vision: We offer 2 options for our benefit-eligible Team Members administered by VSP.
Medical Spending Accounts: Depending on the Medical Plan selected, Team Members may enroll in a Flexible Spending Account (FSA) or a Health Savings Account (HSA). These plans allow Team Members to use pre-tax dollars for medical expenses.
Dependent Care Accounts: Benefit-eligible Team Members may sign up for pre-tax savings towards day care expenses.
Diabetes Program: Allows Team Members and their covered dependents to receive discounts on benefits and meet with a counselor to discuss nutrition, medication and lifestyle issues that affect diabetes.
Short Term Disability: pays Team Members up to 60% of their monthly salary for up to 6 months for each qualifying disability event. This is disability coverage for illness, injury or accident.
Long Term Disability: pays Team Members up to 60% of their monthly salary after they have been out of work for more than 6 months. This is disability coverage for illness, injury or accident.
Long Term Care: The type of assistance is needed if you are unable to care for yourself because of a prolonged illness or disability, from daily activities at home to skilled care in a nursing home. This is available to benefit-eligible Team Members, their spouses, parents (including in-laws) and grandparents (including in-laws).
Basic Life Insurance and Basic Accidental Death & Dismemberment: Company provides both these benefits with no cost to Team Members in the amount of the annual salary (up to $50,000 for each plan).
Supplemental Insurance: Eligible Team Members can enroll in supplemental Life or AD&D insurances for themselves, spouse and children.
Retirement Savings Plan: Once you have worked one year and 1,000 hours in that year, you will be eligible for company contributions to the retirement plan starting the following July 1st or January 1st.
Adoption Assistance: Adoption assistance is available to benefit-eligible Team Members for up to $5,000 per adoption for qualified legal and adoption agency expenses as limited in the policy.
Employee Assistance Program (EAP): Provides benefit-eligible Team Members and their covered dependents with confidential counseling services, as well as Mental Health & Substance Abuse services. The first 12 visits are free each plan year.
Team Member Emergency Fund: All paid MSHA Team Members are provided with a confidential mechanism for seeking financial assistance when confronted with personal emergencies.
Bereavement Leave: Full-time, benefit-eligible Team Members receive 3 days off for the death of an immediate relative and one day off for the death of a grandparent or grandchild after 90 days of employment. Part-time, benefit-eligible Team Members receive one day off for the death of an immediate relative after 90 days of employment.
Paid Time Off: Paid time off is offered to benefit-eligible Team Members for rest and relaxation. The amount of time will be accrued annually by Team Members. Accrued, unused time may be paid out at termination of employment if employed more than 90 days.
Major Medical Leave: MML is provided to benefit-eligible Team Members for protection against the potentially negative financial impact of absence due to personal illness or injury. The amount of time will be accrued annually.
PTO Cash-Back Option: Benefit-eligible Team Members have the opportunity to receive cash or retirement funds in lieu of some accrued paid time off once they have 120 hours in their bank.
PTO Donation Plan: Benefit-eligible Team Members with a minimum of 120 hours in their PL/PTO accrual bank may donate hours to Team Members in crisis. Donations can be made to the Team Member Emergency Fund or directly to a Team Member using the PTO cash-out form.
Leave of Absence: Available to benefit-eligible Team Members after designated amount of time for medical, maternity, FMLA, personal and military leave.
Child Development Centers:
On-site child care is available at both JCMC and SSH. Team Members receive highest priority for available openings.
Wellness Center: Serves the community as well as Team Members and their families, offering cardiovascular equipment, free weights, circuit weight training, equipment, variety of fitness classes, heated pool therapy pool, saunas and indoor lap pool.
Tuition Reimbursement: After six months of employment, benefit-eligible Team Members are eligible for assistance when returning to school for a graduate or undergraduate degree. Classes must be related to your current job or qualify you for a different job with MSHA. Team Members who benefit from this program will incur a service obligation to MSHA.
Referral/Recruitment Bonus: Eligible positions and the bonus amounts for each position are published on the MSHA and HR intranet home pages and are updated regularly.
Mountain States Corporate University: All facilities offer programs sponsored by the MSHA University to provide Team Members with professional development opportunities in many professions and occupations.
Christmas Gift Certificates: Provided annually to all Mountain States Team Members at their facility's Christmas party.
Discounted Meals: Available to all Mountain States Team Members who have a cafeteria in the facility in which they work.
Credit Union: Team Members are eligible to become members in the credit union immediately upon employment with Mountain States. Online banking is available.
Service Awards: Beginning at five years of continued service and then at five-year increments, Team Members are recognized at an annual program and presented with a service pin. Stars Program: Team Member recognition program designed to spotlight top-performing individuals and teams. Star Cards are redeemed for prizes.
Online Classified Advertisements: Team Members can advertise items for sale or rent, wanted items and professional services and may also look for items to buy online using the Mountain States intranet.
Fit4Life: As part of the MSHA Fit4Life Program, MSHA team members will receive FREE health screenings to help them become healthier through awareness, education and resources.
Discounts to local businesses: A variety of discounts are available and updated regularly online. Some discounts include movie tickets, Biltmore Estate tickets, dinner shows in Pigeon Forge, cell phones and computers.
Pet Insurance: Voluntary benefit for benefit-eligible team members allows planning for pet's healthcare.
Group Accident Coverage: Provided by Aflac and underwritten by Continental American Life, this insurance is available with guaranteed issue. This insurance offers coverage for issues such as fractures, dislocations, hospital admission & confinement, ambulance, emergency room and many more medical situations. This coverage is portable.
Group Critical Illness: Provided by Aflac and underwritten by Continental American Life, this insurance plan goes into effect immediately-with no medical questions. This insurance offers coverage for issues such as heart attack, stroke, cancer, coma, burns and many other medical disabilities. This coverage is portable.
Whole Life Insurance: Provided through UNUM, this benefit is designed to provide death benefits to your beneficiaries if you pass away, but can also build cash value that you can utilize while still alive. Team Members can enroll spouses, children and grandchildren in their policy. This coverage is portable.
Voluntary Short Term Disability: Provided through Colonial Life, this insurance will replace a portion of the income lost during a disability occurrence. This coverage is portable.
Travelers Auto and Home Insurance: Provided through Travelers Insurance, this plan offers special rates that are not available to the general public.
Legal Insurance: Provided by LegaLease, this insurance allows you to access a nationwide network of attorneys that will offer many free and discounted services. These are some of the covered services: name changes, home sale/purchase, uncontested adoption/guardianship, tax audit-and many more!
Purchasing Power: Allows Team Members to purchase new, brand-name computers, electronics and home appliances through payroll deductions. There is no credit check, no down payments and 12 month payment plan.
