Leave of Absence/Medical Leave

The Wellness Center offers all members the opportunity to take a leave of absence during the course of their membership. A leave of absence is allowed up to 4 months in a calendar year. All members must fill out a leave of absence form and pay a holding fee during this period of time.
Medical Leave:
If your health requires you to take a medical leave, you must bring a note from your physician with the dates of your absence and fill out a medical leave of absence form. There is no charge for your membership during your medical leave.
